Data as the Lifeblood of Stronger, Digitally-connected Integrated Care Systems

A healthy flow of high-quality data has been crucial in tackling Covid-19 and will be key to delivering the 21st century vision for digitally driven, citizen-centred integrated care. The pandemic is a powerful example of the need to combine fragmented data sources from across health, social care, industry, research, and governments to inform decisions. Before fulfilling the potential of digital and data to transform health services and connect patients and providers, there are complex challenges we still need to overcome around interoperability, organisational silos, trust and governance. This session will highlight innovative integrated care initiatives from around Europe that have tackled these issues to enable better population health, more personalised care, improved patient outcomes and reduced workloads.
